Wow, looks really good.

My 3 cents:
1. "Active" should be checkbox.
2. "Activate by default" in this design could be set to 'true' in several 
lists.
3. "Use as fallback" -- what is it?

IF ...
ELSE IF ...
ELSE ...

seems to be a good thing too. And we might get all the config in one line (as 
Evolution does), but it'll be a really long one, so I'm not sure that we 
should do it.
